- Applicant is in need of medical assistance that is medical in nature, such as payment of hospital bills, checkups, prescribe medication, etc.
REQUIREMENTS:
- Filled up loan application forms
- Medical records or pertinent documents as proof for application
- Complied necessary requirements per type of pension
